A general questionnaire provides a starting point; the final participant questionnaire is tailored to the client's needs, participants, and engagement. Each participant receives a personal Confidentiality Agreement; the company receives an agreement confirming that information shared between CSI and individual participants remains confidential and will not be disclosed to the company. Use this form for the engagement overview. Individual questionnaire responses are collected separately. Fees are based on the scope, preparation, session format, and number of participants. A fee will be provided after the request is reviewed.
